“The Rh Blood Group System – Meet the 5 Antigens That Matter Most
You know Rh+ and Rh−.
The Rh system has over 50 antigens — but D, C, c, E, and e are the 5 that account for most clinically significant transfusion complications.
D Antigen
- Frequency: ~85% of population
- Antibody: Anti-D (IgG)
- Most immunogenic Rh antigen
- Causes: severe HTR and HDFN
- Defines Rh+ vs Rh−
C Antigen
- Frequency: ~70%
- Antibody: Anti-C (IgG)
- Causes: mild-moderate HTR and HDFN
c Antigen
- Frequency: ~80%
- Antibody: Anti-c (IgG)
- Causes: HTR and HDFN
- Second most clinically significant after anti-D
E Antigen
- Frequency: ~30%
- Antibody: Anti-E (IgG)
- Causes: HTR and HDFN
e Antigen
- Frequency: ~98% — most common Rh antigen!
- Antibody: Anti-e (IgG) — rare but significant
- Causes: HTR and HDFN
Immunogenicity Order — Strongest to Weakest: D, E, C, c, and e
Key Rule:
All Rh antibodies are IgG, react best at 37°C, and should be considered potential causes of hemolytic transfusion reactions and HDFN.”
