“The Rh Blood Group System – Meet the 5 Antigens That Matter Most

You know Rh+ and Rh−.

The Rh system has over 50 antigens — but D, C, c, E, and e are the 5 that account for most clinically significant transfusion complications.

D Antigen

  • Frequency: ~85% of population
  • Antibody: Anti-D (IgG)
  • Most immunogenic Rh antigen
  • Causes: severe HTR and HDFN
  • Defines Rh+ vs Rh−

C Antigen

  • Frequency: ~70%
  • Antibody: Anti-C (IgG)
  • Causes: mild-moderate HTR and HDFN

c Antigen

  • Frequency: ~80%
  • Antibody: Anti-c (IgG)
  • Causes: HTR and HDFN
  • Second most clinically significant after anti-D

E Antigen

  • Frequency: ~30%
  • Antibody: Anti-E (IgG)
  • Causes: HTR and HDFN

e Antigen

  • Frequency: ~98% — most common Rh antigen!
  • Antibody: Anti-e (IgG) — rare but significant
  • Causes: HTR and HDFN

Immunogenicity Order — Strongest to Weakest: D, E, C, c, and e

Key Rule:

All Rh antibodies are IgG, react best at 37°C, and should be considered potential causes of hemolytic transfusion reactions and HDFN.”
