“Transfusion Week – Case 7 Challenge!
Scenario:
During a neonatal blood transfusion, after 2 hours only 2 mL had infused (expected ~14 mL).
Pump was replaced → transfusion finally completed in total 6.25 hours.
Root cause: Wrong administration set used, leading to pump malfunction.
Questions:
1. What went wrong exactly?
2. Common reasons for this error?
3. What protocols should prevent it?
