Minor ischemic strokes may appear “mild,” but up to 30% of patients develop disability within 90 days — a reminder that minor does not mean benign.
Our guidance provides a pragmatic, scenario-based framework to support clinicians in navigating some of the most controversial areas in acute stroke care:
- Disabling vs nondisabling minor stroke
- Role of IV thrombolysis and dual antiplatelet therapy
- Early anticoagulation in cardioembolic minor stroke
- When (and whether) to perform thrombectomy in low NIHSS with LVO
- Managing rapidly improving symptoms
- Post-reperfusion antithrombotic strategies
- Considerations for low-resource settings
This document aims to translate complex evidence into clear, actionable decision pathways, highlighting where we do have robust data — and where future trials are urgently needed.
