Francesca Palandri: Is There Still a Place for Splenectomy in ITP in the Modern Era?

Francesca Palandri, Hematologist at IRCCS Sant’Orsola Polyclinic – Bologna University Hospital, shared a post on LinkedIn:

“Is there still a place for splenectomy in ITP in the modern era?

With TPO-RAs and rituximab available, splenectomy is often seen as a thing of the past.

In the GIMEMA ITP1324 study, we analyzed a multicentre cohort of 97 adults with ITP who had a splenectomy between 2010 and 2022.

Our data were presented at SIE – Italian Society of Hematology and suggest the answer is yes:

Still effective: 59–69% overall response in the first 6 months (CR up to 65%), despite use after >4 lines of medical therapy.
A safe procedure with modern prophylaxis: 94% of patients were vaccinated, prior rituximab and the surgical approach did not change response or complications.
Choosing patients matters: patients ≥60 years responded less often and needed further treatment almost twice as often (62% vs 37%).

Splenectomy is still a valid option in today’s ITP treatment pathway.”
