Managing Blood Pressure (BP) in acute stroke is a balancing act.

To see how the experts handle it, I have conducted a comparative analysis of these four major guidelines:

  1. ​American Heart Association / American Stroke Association (AHA/ASA)
  2. European Stroke Organisation (ESO)
  3. World Stroke Organization (WSO)
  4. International Society of Hypertension (ISH)

Why do these numbers matter?

As shown in the attached table, the targets change drastically based on the type of stroke:

Ischemic Stroke (The Blockage):
We often allow higher BP. The brain needs extra pressure to push blood past the clot to keep tissue alive.

Hemorrhagic Stroke (The Bleed):
We must lower BP. High pressure acts like a pump, worsening the bleeding.

Thrombolysis Patients:
If we use “clot-busting” drugs, strict BP control is needed to prevent the blockage from turning into a bleed.
