”BP Management in Stroke Care: A major 4-Guideline Comparison

Managing Blood Pressure (BP) in acute stroke is a balancing act.
To see how the experts handle it, I have conducted a comparative analysis of these four major guidelines:
- American Heart Association / American Stroke Association (AHA/ASA)
- European Stroke Organisation (ESO)
- World Stroke Organization (WSO)
- International Society of Hypertension (ISH)
Why do these numbers matter?
As shown in the attached table, the targets change drastically based on the type of stroke:
Ischemic Stroke (The Blockage):
We often allow higher BP. The brain needs extra pressure to push blood past the clot to keep tissue alive.
Hemorrhagic Stroke (The Bleed):
We must lower BP. High pressure acts like a pump, worsening the bleeding.
Thrombolysis Patients:
If we use “clot-busting” drugs, strict BP control is needed to prevent the blockage from turning into a bleed.
