Abdulrahman Al-Mashdali: Targeted Therapies for CNS Involvement in CLL

Abdulrahman Al-Mashdali, Hematology Chief Fellow at Hamad Medical Corporation, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Therapeutic Advances in Hematology, adding:

”I’m pleased to share our newly published systematic review in Therapeutic Advances in Hematology:

‘Novel targeted therapies for central nervous system involvement in chronic lymphocytic leukemia: A systematic review.’

CNS involvement in CLL/SLL is rare, diagnostically challenging, and historically associated with poor outcomes. In this systematic review, we analyzed 26 studies including 32 patients treated with novel targeted therapies.

Key findings:

  • 94.1 percent overall response rate across 34 treatment regimens
  • 55.9 percent complete responses and 38.2 percent partial responses
  • Ibrutinib was the most frequently used targeted agent
  • Venetoclax and ibrutinib–venetoclax combinations demonstrated encouraging activity
  • Responses were observed even in patients with adverse biological features such as TP53 abnormalities, del(17p), and unmutated IGHV

While these findings are encouraging, the evidence remains largely based on case reports and small series. Prospective multicenter studies are needed to better define treatment sequencing, combination strategies, and the role of intrathecal therapy and radiotherapy.
