As a hematologist deeply passionate about women’s health, it is an absolute privilege to collaborate on this review published in Frontiers in Reproductive Health alongside Dr. Manuel Muñoz—undoubtedly one of the most experienced and visionary gynecologists in regenerative medicine applied to assisted reproduction worldwide.
From a hematological perspective, stem cell biology and hematopoietic progenitor mobilization offer remarkable potential for tissue restoration.
Bringing this approach into reproductive medicine allows us to shift the paradigm: moving beyond the focus on individual follicles to repairing and revitalizing the entire ovarian microenvironment and niche.
Developing this review opens the door to compelling new hypotheses that we are already actively researching. Ultimately, our work is driven by a commitment to rigorous science and a shared goal of creating real possibilities for women on their journey to motherhood.

“What if we have been looking at ovarian rejuvenation from the wrong perspective?
I’m very pleased to share our new review published in Frontiers in Reproductive Health.
For years, ovarian regenerative medicine has largely focused on one question:
Can we stimulate the remaining follicles?
Perhaps we should be asking another:
Can we restore the biological ecosystem that allows those follicles to function?
This is the central idea of our work: moving from a follicle-centered to an ovarian niche-centered paradigm.
Ovarian aging is not simply follicular depletion. It also involves progressive changes in the surrounding microenvironment: fibrosis, vascular dysfunction, chronic inflammation, extracellular matrix remodeling and altered intercellular signaling.
From this perspective, we explore whether local regenerative signaling, such as intraovarian PRP, could eventually be complemented by systemic regenerative mechanisms, including G-CSF-induced hematopoietic progenitor cell mobilization.
Importantly, this remains a hypothesis-generating strategy, not an established treatment. Current randomized evidence does not support routine clinical implementation of intraovarian PRP, and rigorous mechanistic and clinical validation is still required.
But our work raises a fascinating question:
Is PRP biologically the same before and after G-CSF-induced progenitor cell mobilization?
We simply do not know.
And this is no longer just a theoretical question for us:
We are currently investigating it.
No conclusions yet — just the next experimental question arising directly from this work.
Perhaps the future of ovarian rejuvenation will depend less on trying to activate a follicle and more on understanding — and eventually restoring — the ecosystem that sustains it.
