Francesca Palandri: Dynamic, Treatment Response-Driven Risk Assessment in Polycythemia Vera
Francesca Palandri, Hematologist at Sant’Orsola-Malpighi Polyclinic – Bologna University Hospital, shared a post on LinkedIn about a recent article she and her colleagues co-authored, published in Blood Advances, adding:
“It’s time to move from static risk assessment to a dynamic, treatment-response-driven approach in Polycythemia Vera.
A new Italian multicenter study coordinated by IRCCS Sant’Orsola Polyclinic – Bologna University Hospital introduces PV-RR6 — a new dynamic prognostic model integrating three readily available variables assessed within the first 6 months of therapy:
- Ruxolitinib dose less than 10 mg BID
- Failure to achieve at least a 50% spleen length reduction
- Persistent phlebotomy requirement
The model identifies three risk categories with clearly distinct 5-year EFS: 89% (low), 71% (intermediate), and 39% (high risk).
Ruxolitinib underdosing (less than 10 mg BID) emerged as an independent prognostic factor for worse event-free survival.
- Underdosing at one timepoint – nearly 2 times higher risk of adverse events (HR 1.94)
- Underdosing at every timepoint – over 5 times higher risk (HR 5.63)
- The most common trigger was mild anemia (Hb 10–12) – for which the label does NOT recommend dose reduction.
Takeaway: In PV, as in myelofibrosis, underdosing isn’t conservative — it’s a risk factor.
PV-RR6 flags high-risk patients early enough to act: optimize dose, switch, or consider clinical trials.
