Irene Cuppari: Whole Blood and Component Therapy in Traumatic Hemorrhage

Irene Cuppari, Director of Transfusion Medicine Service ASST Ovest Milanese, shared Blood Transfusion Journal‘s post on LinkedIn about a recent article she and her colleagues co-authored, published in BT, adding:

“I am pleased to share this Commentary recently published in Blood Transfusion, focused on the use of whole blood in the treatment of traumatic hemorrhage in the prehospital setting.

The recent SWiFT and TOWAR trials represent an important milestone, as they introduce phase 3 randomized evidence in a field that has so far been supported mainly by observational data and physiological rationale.

The message that emerges is clear, though not definitive: whole blood did not demonstrate superiority over component therapy in the prehospital strategies evaluated, but this should not be interpreted as evidence of equivalence or non-inferiority.

A crucial methodological and organizational debate therefore remains open: every transfusion strategy should be assessed not only in terms of clinical effectiveness, but also with regard to sustainability, appropriateness, accessibility, and impact on transfusion services.
