Muhammad Ahsan: Advancing Evidence in Antithrombotic Therapy After STEMI

‘Triple Antithrombotic Therapy versus Dual Antiplatelet Therapy Alone Following Anterior STEMI: A Systematic Review and Meta-Analysis With GRADE Assessment’. Sage Sage Journals
This project is particularly meaningful to me as I had the opportunity to serve as the first author and lead the study from conceptualization and methodology through data analysis, interpretation, and manuscript preparation.
We synthesized evidence from 7 studies involving 3,692 patients with anterior STEMI undergoing primary PCI, addressing an important clinical question: does adding prophylactic anticoagulation to dual antiplatelet therapy meaningfully reduce the risk of left ventricular thrombus, and does that potential benefit justify the additional bleeding risk?
Our analysis found that triple antithrombotic therapy was associated with more than twice the risk of bleeding (RR 2.10, 95% CI 1.60-2.76), while reductions in left ventricular thrombus and other ischemic outcomes were not statistically significant.
Importantly, several estimates remained imprecise, highlighting the limitations of the current evidence base rather than simply demonstrating ‘no benefit.’
For me, the most valuable part of this project was learning that evidence synthesis is not simply about obtaining a statistically significant result.
It is about understanding study design, assessing risk of bias, evaluating certainty of evidence, and translating uncertainty into clinically meaningful conclusions.
I’m grateful to my co-authors and mentors for their contributions and guidance throughout this project.
