Muhammad Ibrahim on Efficacy and Safety of Extended DOACs Use in VTE

”Excited to co-author this amazing systematic review and meta-analysis entitled: Efficacy and Safety of Direct Oral Anticoagulants for Extended Treatment of Venous Thromboembolism.

Clinical question: Should we continue DOACs long-term after initial VTE treatment?

Analysis of 5,600+ patients from RCTs showed:

83% reduction in VTE recurrence
62% reduction in all-cause mortality
No significant increase in major bleeding
Higher clinically relevant non-major bleeding

Bottom line: In properly selected, low-bleeding-risk patients, extended DOAC therapy provides a clear net clinical benefit for secondary prevention of VTE.
