Nethuki Kulasinghe: Sequential Anticoagulation in Postpartum Iliocaval Thrombosis With Submassive PE and HIT

“Excited to share that I co-authored a new publication in SN Comprehensive Clinical Medicine, part of Springer Nature.
Our case report describes a rare and complex case of postpartum iliocaval thrombosis complicated by submassive pulmonary embolism and heparin-induced thrombocytopenia (HIT) in a patient with underlying protein C deficiency. The case highlights the successful use of sequential anticoagulation with rivaroxaban and apixaban after failure of heparins and fondaparinux.
This work contributes to the limited literature on HIT in the postpartum period and underscores the importance of early recognition, multidisciplinary management, and timely transition to non-heparin anticoagulation in life-threatening thromboembolic disease.
