Persistent Postoperative Anemia: A Prognostic Red Flag?

A recent, well-conducted retrospective study by Choi et al., published in Anaesthesia Journal, caught my attention:

  • 𝗠𝗼𝗿𝘁𝗮𝗹𝗶𝘁𝘆 𝗶𝗻𝗰𝗿𝗲𝗮𝘀𝗲𝗱 𝟭𝟰.𝟱-𝗳𝗼𝗹𝗱 in patients who developed anemia after valvular heart surgery.
  • Those with persistent anemia faced an 11-fold increase.
  • In contrast, patients without anemia—before or two months after surgery—had significantly better outcomes.

These findings underscore the underappreciated prognostic value of monitoring hemoglobin recovery post-surgery. But here’s the nuance:
“The findings should not be assumed to imply that correcting postoperative anemia will improve patient survival.” This is a hypothesis-generating signal, not a call to action—yet. It highlights the urgent need for prospective studies to determine whether intervention can truly improve long-term outcomes—and what those interventions should be.”
