‘Antiphospholipid Syndrome (APS): when “unprovoked” clots and pregnancy loss connect

What it is: An autoimmune pro-thrombotic state causing venous/arterial thromboses and pregnancy morbidity.

It can be primary or overlap with SLE.

Patients often appear “too young” for DVT/PE or stroke; recurrent miscarriage may be the first clue.

Clinical clues: unprovoked DVT/PE, stroke/TIA under 50, livedo reticularis, thrombocytopenia, recurrent early losses or late complications in pregnancy.

Management changes dramatically if APS is confirmed—anticoagulation choices and obstetric planning differ.

Diagnostics (snapshot):
– Antibodies: anticardiolipin (aCL) and anti-β2GPI (IgG/IgM).
– Functional: lupus anticoagulant (LA).
– Key principle: persistence—positive results repeated after ~12 weeks to confirm.”
