”Rethinking lipid management for stroke prevention – new ILEP Consensus Paper
Ischemic stroke accounts for ~66% of all strokes and is now the third leading cause of death worldwide – yet a large proportion of patients still have undiagnosed or undertreated lipid disorders, both before and after a cerebrovascular event.
Together with an amazing international team from the International Lipid Expert Panel (ILEP), in collaboration with the ESC Council on Stroke, we have just published a new consensus paper on lipid management for primary and secondary stroke prevention in Progress in Cardiovascular Diseases
What’s inside the paper?
- Why dyslipidaemia (including LDL-C and Lp(a)) is a causal driver of ischaemic stroke
- Clear, pragmatic LDL-C and apoB targets for patients at risk of stroke and for those with prior stroke/TIA
- A strong message in favour of early, intensive and often combination lipid-lowering therapy (statins + ezetimibe ± PCSK9 inhibitors, bempedoic acid, inclisiran) to close the huge treatment gap in real-world practice
- How to think about Lp(a), triglyceride-rich lipoproteins, and non-HDL-C in stroke prevention
- The ILEP-SMILE framework: simple lifestyle “tips for the healthy cerebrum” to integrate pharmacological and non-pharmacological prevention
Our message is simple – and fully implementable in everyday practice:
“Lower is better, earlier is better, and longer is better” for LDL-C and overall lipid management in patients at risk of stroke and after stroke.
Even partial, consistent implementation of these principles could translate into fewer strokes and better long-term outcomes for our patients.”
