“Lupus anticoagulant: an old concept revisited with new mechanistic insight
Recent reflections by Nicola Pozzi remind us that some of the most established concepts in hemostasis and thrombosis still warrant critical re-examination.
In collaboration with Vittorio Pengo, this work revisits lupus anticoagulant from both a historical and mechanistic perspective, integrating emerging insights on antiprothrombin antibodies that have accumulated over recent years.
Why this matters
• Lupus anticoagulant remains a cornerstone laboratory criterion in the diagnosis of antiphospholipid syndrome
• Current testing strategies do not always reflect the underlying biological mechanisms
• New mechanistic data challenge how we interpret and even define what we measure in routine coagulation assays

Is it time to reconsider—and potentially rewrite—parts of our coagulation textbooks?
