Piotr Czempik: Viscoelastic Testing Guiding Lumbar Puncture in Waldenström’s Macroglobulinemia
Piotr Czempik, Chair of Department of Anesthesiology and Intensive Care at Medical University of Silesia, shared on LinkedIn about a recent article he and his colleague co-authored, adding:
”Viscoelastic testing to guide lumbar puncture in Waldenström’s macroglobulinemia
Patients with Waldenström’s macroglobulinemia often have unpredictable bleeding risks — especially when treated with BTK inhibitors.
In our recent case, a patient with thrombocytopenia (PLT 58 × 10⁹/L) required a lumbar puncture.
Traditional coagulation tests offered limited clarity.
Using ROTEM, we assessed global hemostasis and found preserved platelet contribution to clot strength, despite low PLT count and BTKi therapy.
Based on this, we proceeded without prophylactic platelet transfusion, and the procedure was complication‑free.
Key message:
In complex hematologic conditions, platelet count alone may be misleading. Viscoelastic hemostasis assays can support safer, more individualized procedural decisions.”
