“Washed PRBCs – Precision Beyond Compatibility
Not all transfusion reactions are immunohematologic incompatibilities.
Sometimes, the culprit is residual plasma.
Washed PRBCs are red cell units processed to remove:
- Plasma proteins
- IgA
- Cytokines
- Bioactive mediators
Indicated in:
- Severe IgA deficiency with anti-IgA
- Recurrent severe allergic transfusion reactions
- Selected neonatal exchange transfusions
Limitations:
- 24-hour shelf life (post-processing)
- RBC loss during washing
- Increased cost and labor
Clinical Insight: Washed red cells are not about matching better —they are about transfusing less of what should not be there.”
