Abdul Mannan: Optimizing Eltrombopag Response Through Proper Timing
Abdul Mannan, Consultant Haematologist at Betsi Cadwaladr University Health Board, shared a post on LinkedIn:
“One glass of milk can cancel your eltrombopag dose.
Here’s why.
Eltrombopag doesn’t fail patients.
Bad timing does.
This is one of the most commonly missed drug-food interactions in haematology practice.
And it matters more than most people realise.
Eltrombopag chelates polyvalent cations.
Calcium, magnesium, iron, aluminium.
When these bind to the drug in the gut, absorption drops.
The platelet count doesn’t rise.
And the team wonders why treatment isn’t working.
Here’s the rule patients need to remember:
- Take it once daily, at the same time every day
- Take it on an empty stomach or with a low-calcium meal (50 mg calcium or less)
- Wait at least 2 hours before calcium-rich food, antacids, or mineral supplements
- Or wait 4 hours after if you’ve already eaten or taken them
- Dairy, antacids, multivitamins, and calcium-fortified drinks all count
Safe options at the time of dosing: water, unfortified juice, plain toast, rice, fruit, or lean meat.
Missed dose?
Take it at the usual time next day.
Never double up.
The SmPC for eltrombopag clearly states this interaction, yet in clinical practice, it’s underappreciated.
A patient taking eltrombopag with morning yoghurt and a multivitamin is essentially wasting half their dose.
Timing is therapy.
