“Case study: The APTT was 96 seconds.

The patient had no history of bleeding.

So what would You do next?

A 72-year-old patient arrives with:

Extensive spontaneous bruising, with painful muscle haematomas.

There is no previous bleeding disorder and not on anticoagulant.

The coagulation screen shows:

  • PT: Normal
  • APTT: 96 sec (significantly prolonged)

Now here’s the question for you

  1. What’s your First thought?
  2. Heparin contamination?
  3. Lupus anticoagulant?
  4. Factor deficiency?
  5. Acquired inhibitor?

This is where coagulation becomes detective work.
