Irma Bagdoniene: The Silent Progression of Coronary Artery Disease

“Sunday Story from the Lipid Clinic (Part 2): The Story Behind the Numbers.
Last week, I shared the case of a patient with:
- LDL-C 190 mg/dL
- Lipoprotein (a) 100 mg/dL
- HbA1c 7.6 percent
The coronary CT showed plaques, but one question remained: How advanced was the disease?
This week’s coronary angiography (Dr. med. Mohammad Al Mawed) gave the answer.
The most striking finding was a hemodynamically significant 80 percent left main coronary artery stenosis (minimal lumen area: 5.2 mm²), accompanied by severe multivessel coronary artery disease, leading to the recommendation for coronary artery bypass graft surgery.
The patient has now been referred for coronary artery bypass graft surgery.
The important message is not that this patient needs bypass surgery.
The important message is that the laboratory values already told the story before the angiography did.
LDL-C of 190 mg/dL, elevated lipoprotein (a), and diabetes had been silently driving atherosclerosis for years.
The coronary angiography simply revealed the consequence.
As cardiologists, we often celebrate successful PCI or CABG.
But our greatest success is preventing patients from ever needing them.
