Reversing the effects of warfarin:

  • Vitamin K (phytomenadione)

Promotes synthesis of clotting factors II, VII, IX, X.

Oral: slower onset, used for non-urgent reversal.

IV: faster onset, used for significant bleeding.

  • Prothrombin Complex Concentrates (PCCs)

Contains concentrated clotting factors II, VII, IX, X.

Rapid reversal, especially in life-threatening bleeding.

Preferred over fresh frozen plasma for speed and lower volume.

  • Fresh Frozen Plasma (FFP)

Provides clotting factors directly.

Slower and requires larger volume compared to PCC.

Used when PCC is unavailable or in certain clinical settings.

  • Recombinant Activated Factor VII (rFVIIa)

Activates clotting cascade to bypass warfarin effect.

Used in critical cases; off-label.

Limited by high cost and thrombotic risk.”
