Jared Rice: Understanding Extreme Platelet Counts in Preterm Infants

“An extreme platelet count on a preterm infant’s CBC gets read two different ways depending on who’s looking at it. One clinician treats it as an incidental finding and moves on. Another orders a hematology workup for a possible marrow disorder. Most of the time, both are overcorrecting in opposite directions.

The count itself does not tell you which response is correct. A falling hemoglobin on the same panel, the week of life the count appeared, and how it compares to what preceded it carry more diagnostic weight than the platelet number alone, and that context is exactly what gets skipped when a lab value this high triggers a reflexive reaction.

There is also a rare look-alike hiding inside the range that accounts for nearly every case, and it does not announce itself with a higher number. It shows up through persistence and pattern instead.
