Wolfgang Miesbach: Effectiveness of Avatrombopag in ITP Treatment

Wolfgang Miesbach, Head of the Hemostasis and Hemophilia Center at Department of Medicine II, University Hospital Frankfurt, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in TH Open, adding:

“Our European real-world study on avatrombopag in immune thrombocytopenia is now published in TH Open, open access.

Randomised trials have shown that avatrombopag is effective in chronic ITP.

What we wanted to know is how it performs in routine care, across different health systems, and in patients who had already been through another TPO receptor agonist.

We reviewed the records of 76 adults with primary ITP treated at 16 centres in Belgium, the Czech Republic, Germany, Italy, Spain and the UK, with a median follow-up of 16 months.

93% reached a platelet count of equal to or more than 50 × 10⁹/L by week 12.

87% had a complete response (equal to or more than 100 × 10⁹/L) at least once by week 52.

Responses came quickly, with a median of 10 days to equal to or more than 50 × 10⁹/L and 19 days to equal to or more than 100 × 10⁹/L.

Of the 26 patients on corticosteroids at initiation, 25 were able to stop or reduce them.

Nineteen patients (25%) stopped avatrombopag permanently.

The commonest single reason was a maintained response with a view to sustained remission off treatment (7 patients).

All seven had reached a complete response beforehand, and none required new treatment over a median of 366 days off therapy.

Four thromboembolic events occurred in three patients, a rate of 4.9 per 100 patient-years.

The confidence interval is wide (1.3–12.5) and rests on very few events, so the figure should be read with caution.

ITP-related inpatient admissions were infrequent, at 0.1 per person-year.

Time-specific analyses showed that platelet counts fluctuate considerably in real-world practice, a reminder that a single response threshold at one visit tells only part of the story.
