On Sept 16, 2026, the American Society of Hematology (ASH) released new guidelines with higher serum ferritin cutoffs for diagnosing iron deficiency:
- Children (9 months to 4 years): ≤ 20 ng/mL
- Adults, menstruating & pregnant individuals: ≤ 30 ng/mL
- High-risk groups (heavy menstrual bleeding, pregnant with anemia): ≤ 50 ng/mL
- Adults with inflammation (cancer, IBD, infection): < 100 ng/mL or TSAT < 20%
Why it matters: iron deficiency affects ~2 billion people worldwide, and up to 70% of cases in high-risk groups are missed. Higher thresholds mean earlier diagnosis.
