Mary Cushman: Antiphospholipid Antibodies and Their Association with Ischemic Stroke

Mary Cushman, President Elect at International Society on Thrombosis and Haemostasis (ISTH), shared a post on LinkedIn about a recent article she and her colleagues co-authored, published in Stroke, adding:

“Antiphospholipid antibodies and stroke

Lots of interesting findings in this new study I helped out with. Anything surprising to you?

  • 52 studies (40 case-control, 12 cohort).
  • Prevalence of aPL seropositivity in 9438 patients with ischemic stroke: 19.1% (95% CI, 15.4%-23.0%). That’s high!
  • aPL seropositivity associated with higher odds of first ischemic stroke (43 studies, 19 097 patients; OR, 2.93 (95% CI, 2.31-3.73)
  • Strongest associations for lupus anticoagulant: OR, 6.69 (95% CI, 2.94-15.2).

What do we do next?

  • Test all ischemic stroke patients and confirm if they have APS
  • Consider other treatments for these patients? Anticoagulation, immune modulation?
  • If we test broadly, should all APS patients with stroke be anticoagulated? With DOAC or warfarin?

Thankful to Behnood Bikdeli for his leadership on this.”
