“The ABO system is more than just A, B, AB, or O.

Under the microscope, most red cells may look identical…

but immunologically, they can be completely different.

The presence or absence of ABO antigens plays a critical role in:

  • Transfusion compatibility
  • Immune recognition
  • Patient safety
  • Hemolytic risk

An ABO-incompatible transfusion is not simply a ‘blood group mismatch’ —

it can trigger a rapid, severe, and potentially fatal hemolytic transfusion reaction.

Even patients with the same ABO group may still require antibody screening and crossmatching, especially if they have a history of transfusion, pregnancy, or unexpected antibodies.
