Aniela Popescu: Heart Failure Care Enters a New Era With the 2026 ESC Guidelines

Aniela Popescu, Fellow of the European Society of Cardiology at European Society of Cardiology, shared a post on LinkedIn:

“Hot off the press: new 2026 ESC Heart Failure Guidelines from ESC Congress 2026!

A new way of thinking about heart failure, with more emphasis on prevention, phenotyping and individualized treatment.

Some of the key messages:

Simplified HF classification: HFrEF less than 50 percent and HFpEF greater than or equal to 50 percent
Greater focus on prevention and pre-heart failure
A more multiparametric diagnostic approach, integrating biomarkers and multimodality imaging, with greater emphasis on natriuretic peptides
New treatment terminology: Foundational Medical Therapy (FMT) as the therapeutic backbone, complemented by Additional Medical Therapy (AMT) and Guideline-Directed

Interventional Therapy (GDIT)

SGLT2 inhibitors and MRAs as foundational therapy in symptomatic HF, irrespective of LVEF
‘Decompensated heart failure’ replaces ‘acute heart failure’, encompassing both acute and gradual worsening requiring urgent treatment
Greater integration of device and interventional therapies, with CRT potentially considered earlier in appropriate patients

A shift towards earlier recognition, better phenotyping and a more integrated, personalized approach to heart failure care.
