Aravind Palraj: Uncovering the Drivers of Occlusive Retinal Vasculitis in Young Patients

Aravind Palraj, Senior Resident in Clinical Immunology and Rheumatology at Madras Medical College, shared a post on X:

“Occlusive retinal vasculitis in a young adult deserves a structured work-up.

The key question is not just ‘Is this vasculitis?’

It is: what is driving the vascular occlusion?

Important possibilities include:

  • Behçet disease
  • Ocular TB / Eales spectrum
  • Syphilis APS / other thrombotic states
  • Sarcoidosis / ANCA-associated disease / infectious retinitis

Ultra-widefield fluorescein angiography helps define the retinal vascular phenotype and extent of ischemia.

Young age and occlusion should prompt a careful search for inflammatory, infectious, and thrombotic causes.”
