Jean Amiral: Clinical Value of Quantitative PF4-Dependent Antibody Measurement

”Quantitative measurement of PF4 related antibodies in HIT, VITT, and PF4-dependent syndromes.
PF4-dependent antibodies are associated to various diseases, especially HIT when targeted first to heparin-PF4 complexes, but also VITT with a preferential reactivity to PF4, as well they are in the rare PF4-dependent syndromes (out of any heparin therapy or adenovirus vector vaccination).
Quantitative or semi-quantitative measurements of these PF4-dependent antibodies can be of high usefulness for analyzing associations with clinical syndromes. We reported a recombinant humanized monoclonal antibody to HPF4 complexes (code: K070), which behaves as anti-HPF4 symptomatic antibodies developed in HIT, and which has been presented as a poster at ISTH congress 2026 in Paris.
In immunoassays, K070 generates parallel dose-response curves like those obtained with pathogenic HIT antibodies This makes this antibody suitable and a good candidate for calibrating these methods.
In functional assays, a dose-dependent activation of platelets is obtained, still variable with the healthy platelet donor. K070 presents a close reactivity, whether it is tested in buffer or in plasma (little interference from plasma IgGs for binding to FcGRII-A platelet receptors).
In VITT and PF4-associated syndromes, anti-PF4 antibody cross-reactivity with HPF4 can also be quantitated.
This antibody then allows a better evaluation of antibody reactivity and its implications in clinical complications.”
