Mehmet Cilingiroglu: The First Diagnosis of Coronary Thrombosis in a Living Patient
Mehmet Cilingiroglu, Clinical Professor of Medicine/Cardiology at the University of Hawaii John A. Burns School of Medicine, Cardiologist at Hawaii Pacific Health, shared a post on LinkedIn:
“Did you know that the first documented correct diagnosis of coronary thrombosis in a living patient dates back to 1876—nearly 150 years ago?
In Vienna, Austrian physician Dr. Adam Hammer recognized coronary artery occlusion in a 34-year-old man who presented with sudden collapse and profound bradycardia, with the diagnosis subsequently confirmed at autopsy.
His remarkable clinical observation, published in 1878, preceded James B. Herrick’s landmark 1912 description of myocardial infarction by more than three decades.
Interestingly, Hammer’s case involved coronary occlusion associated with infective endocarditis rather than the typical atherosclerotic plaque rupture responsible for most acute myocardial infarctions today.
At a time when electrocardiography, cardiac biomarkers, coronary angiography, and modern cardiovascular imaging did not exist, this diagnosis represented an extraordinary achievement in clinical reasoning.
From bedside observation in 1876 to contemporary primary PCI, intravascular imaging, and advanced cardiovascular technologies, the history of myocardial infarction reminds us how far cardiovascular medicine has advanced—and how every major discovery begins with curiosity, careful observation, and the courage to challenge conventional thinking.”
