When to consider the reduced doses of DOAC??
- One of the most common (and highest-stakes) medication safety checks in practice: is your patient on the RIGHT dose of their DOAC?
- Always take in consideration the following points before deciding the right dose of DOAC for your patient
Clinical indication
Like Atrial Fibrillation or VTE management
Kidney Function
Especially if CrCl is less than 30 mL/min or if Hemodialysis
Comorbid Conditions
Ex. if AF and ACS patient undergoing PCI
Age
Especially if Elderly (80 years or older)
Weight
Especially if 60 kg or less
Drug Drug interactions
Especailly with strong Inhibitors of cyp 3A4 or P-gp
