“Thrombophilia in 2026

The modern approach to thrombophilia emphasizes precision medicine, recommending selective testing only when the results are likely to influence clinical management.

Testing is appropriate for patients with unprovoked or recurrent VTE, thrombosis at unusual sites, recurrent pregnancy loss, a strong family history, or suspected antiphospholipid syndrome, while it is generally not recommended for provoked VTE, during acute thrombosis or anticoagulation, or as routine screening of relatives.

Current evaluation focuses on high-yield tests, including Factor V Leiden, Prothrombin G20210A, antithrombin, protein C and protein S deficiencies, and antiphospholipid antibodies.

Treatment decisions should be based primarily on the patient’s clinical risk rather than thrombophilia status alone.

High-risk thrombophilias often require specialist care and extended anticoagulation, while emerging evidence supports the use of DOACs in selected patients.

Special populations, particularly pregnant women and patients with antithrombin deficiency, require individualized risk assessment and multidisciplinary management.

Overall, thrombophilia testing should be purpose-driven, evidence-based, and tailored to improve patient outcomes rather than performed routinely.”
