Osama Alabdi: Never Ignore a Weak Reaction in the Blood Bank

“Never ignore even a weak reaction in the Blood Bank.
I recently had an interesting case involving an 85-year-old ICU patient who required a blood transfusion.
I started with routine blood typing and antibody screening using the Bio-Rad Laboratories gel card.
Blood Group:
- Anti-A: 4 plus
- Anti-B: 0
- Anti-D: 4 plus
- A Cells: 3 plus
- B Cells: 4 plus
- Antibody Screen: Negative
My first thought was an ABO discrepancy such as an A₂ subgroup.
Anti-A₁ testing was positive so I looked further.
I performed an antibody identification panel using the tube method.
The results were:
- 2 plus at Immediate Spin (IS)
- 3 plus after 15 minutes at room temperature
- Negative at 37 degrees Celsius and AHG
This pattern suggested the presence of a cold antibody.
A negative antibody screen by gel card does not always rule out a cold antibody.
Every reaction has a reason. Don’t ignore it.”
