Washed PRBCs – Precision Beyond Compatibility

Not all transfusion reactions are immunohematologic incompatibilities.

Sometimes, the culprit is residual plasma.

Washed PRBCs are red cell units processed to remove:

  • Plasma proteins
  • IgA
  • Cytokines
  • Bioactive mediators

Indicated in:

  • Severe IgA deficiency with anti-IgA
  • Recurrent severe allergic transfusion reactions
  • Selected neonatal exchange transfusions

Limitations:

  • 24-hour shelf life (post-processing)
  • RBC loss during washing
  • Increased cost and labor

Clinical Insight: Washed red cells are not about matching better —they are about transfusing less of what should not be there.”
