One Donor, One Procedure, One Therapeutic Difference

Today we celebrate Apheresis and Single Donor Platelets (SDP)—where advanced technology, donor commitment and transfusion science come together to support patients when they need.

What is SDP?

  • Platelets are selectively collected from a single donor using an automated cell separator.
  • Other blood components are safely returned to the donor.
  • A therapeutic platelet collection generally targets 3 × 10¹¹ or more platelets in one procedure, depending on the collection system and donor.

Who can be a platelet apheresis donor?

  • A healthy, eligible donor who meets applicable blood-service criteria
  • Appropriate body weight according to local regulations or protocol
  • A suitable, accessible vein for apheresis
  • Aspirin and Clopid not taking particularly drugs affecting platelet function
  • No current illness
  • Adequate platelet count and haemoglobin according to donor criteria
  • Successful donor screening and pre-donation assessment

Donor eligibility must always follow the applicable national amd blood-centre standards.

The platelet increment matters!

In a stable adult without significant platelet consumption, an adequate SDP dose may produce a platelet increment of approximately 20,000–40,000/µL, although 30,000–60,000/µL may be seen in some patients.

The response may be reduced by:

  • Sepsis and fever
  • Active bleeding
  • DIC or consumption
  • Splenomegaly
  • HLA or HPA alloimmunization
  • Immune-mediated platelet destruction

CCI — Corrected Count Increment

CCI equals (Post-transfusion platelet count minus Pre-transfusion platelet count) times BSA (m²) divided by Platelet dose transfused (×10¹¹)

CCI is useful when evaluating poor platelet response and platelet refractoriness.

When are platelets indicated?

  • Clinically significant bleeding
  • Appropriate prophylaxis in severe thrombocytopenia
  • Selected invasive procedures or surgery
  • HLA or HPA-selected platelets when indicated
  • Management of platelet refractoriness

  • Adequate platelet yield or content
  • Appropriate component volume
  • Leukoreduction where applicable
  • 20–24°C storage with continuous gentle agitation
  • Appropriate storage duration
  • Aseptic collection
  • Complete traceability and quality monitoring
