Roctavian, 2.9 million dollar. One treatment, one lifetime, one very fragile factory.

  • Vector manufacturing alone can cost 3–7 million dollar per 500L batch — and a single batch may only treat 2–3 adult hemophilia B patients.
  • The industry still lacks a standardized, commercial-scale AAV/LNP manufacturing platform equivalent to what exists for monoclonal antibodies.
  • The choice of upstream platform — transient HEK293 transfection vs. stable producer cell lines — remains the single most consequential, unresolved manufacturing decision in the field.

This is almost exactly the standardisation gap mRNA/LNP manufacturing faced in 2021 — proven at lab scale, unproven at commercial scale, every developer solving the same problems independently.

Having lived through that phase with mRNA, I’d bet gene therapy manufacturers are about to relearn several of the same lessons the hard way.
