Why does the PLASMIC score for TTP include a “low” MCV?
Because schistocytes are, by definition, smaller than mature RBCs → ↓MCV.

Why is the cutoff in the PLASMIC score <90 fL (not <80, which is lower limit of normal)?
Because the drop is modest (schistocytes are few vs intact RBCs) and because it’s partly offset by reticulocytosis (↑MCV).
