Grégoire Le Gal: Using an Age-Adjusted D-Dimer Cutoff Safely Excluded DVT

“New publication in JAMA

Age-adjusted D-dimer can safely rule out suspected DVT
A large multinational prospective management study (n = 3,205 outpatients) provides strong evidence that age-adjusted D-dimer thresholds are not only valid for PE, but also for suspected lower-limb DVT!

Key findings

Using an age-adjusted D-dimer cutoff (age × 10 µg/L for patients ≥50 years) safely excluded DVT
No venous thromboembolic events at 3 months among patients with D-dimer levels between 500 µg/L and their age-adjusted cutoff
+7.4% absolute increase in the proportion of patients in whom DVT could be ruled out without imaging
Reduces unnecessary compression ultrasonography, same safety.
