The internal mammary artery is frequently used as a graft during coronary artery bypass surgery (CABG), but in some patients, undiagnosed vascular issues may make its use risky.

Performing a CT aortogram in selected patients helps ensure proper conduit selection and prevents potential perioperative complications.

Given the possibility of complex anatomical variations, coronary management should be tailored through a multidisciplinary approach rather than relying solely on standard guidelines.
