Gianluca De Rubeis: A Safer Approach to Antiplatelet Timing in Endovascular Aneurysm Treatment

Gianluca De Rubeis, Interventional Neuroradiologist at San Camillo Forlanini Hospital, shared on LinkedIn about a recent article he and his colleagues co-authored, published in Neuroradiology, adding:

”’The timing of intraprocedural antiplatelet therapy did not affect outcomes in endovascular treatment of ruptured intracranial aneurysms’ — our systematic review is now live.

Here’s the key takeaway: antiplatelet therapy — given to reduce thromboembolic complications — doesn’t need to be administered at the very start of the procedure.

It can be safely delayed until after the first coil is deployed, with no difference in outcomes.

Why does this matter?

Once the first coil is in place, the aneurysm is protected.

Giving antiplatelet therapy at that point keeps the same efficacy against clot complications — while making the overall procedure safer.

A simple but important shift in how we think about timing in the most critical moments of endovascular treatment.
