Jawad Derbas: Comparing Mechanical and Pharmacomechanical VTE Prophylaxis in Spine Surgery

Jawad Derbas, Clinical Fellow at Rizzoli Orthopaedic Institute, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Cureus, adding:

“Pleased to share our newly published paper.

‘Caprini Score-Guided Venous Thromboembolism Prophylaxis in Spine Instrumentation Surgery: A Comparative Analysis of Mechanical vs. Pharmacomechanical Strategies Across Etiological Subgroups.’

Our retrospective study of 265 patients undergoing spine instrumentation surgery evaluated outcomes following mechanical versus combined pharmacomechanical VTE prophylaxis.

Key findings:

  • 212 patients (80%) received mechanical prophylaxis alone, with no postoperative VTE events observed.
  • 53 patients (20%) received combined prophylaxis, with one postoperative PE (1.9%) in a patient with malignancy and pre-existing DVT.
  • No postoperative epidural hematomas were observed in either group.
  • Higher Caprini scores, polytrauma, and previous VTE were associated with the selection of combined prophylaxis.

Conclusion: Our findings suggest that mechanical prophylaxis, supported by early mobilization and selective pharmacological prophylaxis in higher-risk patients, may be associated with low VTE rates following spine instrumentation surgery.

Larger prospective studies are needed to validate these findings.

Grateful to all my co-authors and colleagues for their valuable contributions.”
