Mud Alvi: Restarting Antithrombotic Therapy After Hemorrhagic Transformation of Ischemic Stroke

Mud Alvi, Professor and Vice Chair of Neurology at West Virginia University, shared a post on LinkedIn about a recent article by Seyyed Sina Hejazian et al, publishe in JAHA, adding:

“A patient develops hemorrhagic transformation after an ischemic stroke. When do you restart antiplatelets or anticoagulation?

It’s one of the toughest calls on the stroke service and until now, the evidence has been thin.

Proud to be part of a new multi institutional study in JAHA, led by Ramin Zand looking at 748 patients with hemorrhagic transformation after ischemic stroke.

The finding: timing of restart same day, early, or later showed no significant difference in HT exacerbation, recurrent stroke, vascular death, or functional outcomes.

The exact day may matter less than we think.

The decision to restart, and with what, still has to be individualized.”
