
VTE in Maternal Mortality: ThrombUS+ Horizon Europe Project and Elvira Grandone
“Maternal death during pregnancy, childbirth, or the postpartum period remains a major tragedy for families and communities. Although less than 1% of global maternal deaths occur in high-income countries, it is estimated that approximately 50% of these deaths are potentially preventable.
In line with the Sustainable Development Goals (2016–2030), European countries are also called to action to eliminate avoidable maternal and neonatal deaths. A critical first step is the improvement of data monitoring and reporting, ensuring that no maternal, fetal, or neonatal death goes unnoticed.
Recent data from the Italian Obstetric Surveillance System (ItOSS) revealed significant geographic disparities in maternal mortality across Italy, with higher rates in Southern regions (10.5 per 100,000 live births) compared to the North (7.7) and Center (5.9).
Among maternal deaths occurring within 42 days postpartum, direct obstetric causes accounted for 55.1%, with obstetric hemorrhage being the leading cause (37.1%), followed by sepsis (13.9%), hypertensive disorders (13.4%), and notably, venous thromboembolism (VTE) at 11.9%.
These findings highlight the urgent need for enhanced prevention, early recognition, and management of VTE, a still under-recognized but significant contributor to maternal mortality in Italy.
