Portal hypertension affects thousands of children worldwide, yet diagnosis remains invasive and treatment options are limited. Our comprehensive review examines how immune dysregulation—including altered cytokine profiles, T cell dysfunction, and inflammatory responses—drives disease progression in conditions such as biliary atresia and portal vein thrombosis. By understanding these immune mechanisms, we’re identifying potential non-invasive biomarkers and novel therapeutic targets beyond liver transplantation.
