Carlo Giulioni: Perioperative Management of Antithrombotics in Patients Undergoing Endoscopic Prostate Surgery

Carlo Giulioni, Medical specialist at Villa Igea Nursing Home, shared on LinkedIn about a recent article he and his colleagues co-authored, adding:

”I’m thrilled to share the publication of meta-analysis on the perioperative management of antithrombotic therapy in patients undergoing endoscopic surgery for BPH, recently published in Prostate Cancer and Prostatic Diseases.

In this study, we analyzed 15 studies including 6,091 patients, comparing perioperative outcomes in patients undergoing endoscopic prostate surgery with ongoing antithrombotic therapy versus controls.

Main findings

  • Photoselective vaporization of the prostate (PVP) showed comparable perioperative outcomes, suggesting it may be a safe option when antithrombotic therapy cannot be interrupted.
  • TURP and endoscopic enucleation were associated with higher bleeding-related complications, particularly in patients on anticoagulant therapy.
  • Enucleation procedures also showed higher rates of bleeding requiring surgical hemostasis and acute urinary retention in anticoagulated patients.

Take-home message
Procedure selection is crucial in BPH patients on antithrombotic therapy, and PVP may represent the preferred surgical approach in patients requiring uninterrupted treatment, while decisions should always involve careful multidisciplinary risk assessment.
