João Paulo Marochi Telles: Antibiotic Treatment in Febrile Neutropenia

João Paulo Marochi Telles, ID Specialist – Antimicrobial TDM at A.C.Camargo Cancer Center, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in International Journal of Antimicrobial Agents, adding:

“Our editorial is out in the International Journal of Antimicrobial Agents (IJAA) (with Birgit Koch)!

We did not try to settle the debate about optimizing antibiotics in patients with febrile neutropenia we tried to name where it goes wrong. Why trials don’t demonstrate significant benefit?

Are we even enrolling the right patients, when a large share of febrile neutropenia episodes never yield a bacterial diagnosis?

When clearance is augmented, is ‘prolonged infusion’ really the best we can offer or does it have to be continuous, and for which CrCl range?

Are endpoints like all cause mortality too blunt to detect what these interventions actually change and what would DOOR capture instead?

In the absence of neutrophils, is 100 percent fT higher MIC still the target or should we be discussing exposure up to 4times  MIC?

We do not claim to have resolved these. The editorial is our attempt to put them in the right perspective.”

Thank you Birgit for this colab!
Very nice!”
