A large single-center series (123 cases) from CMC Vellore revisits its role in unexplained splenomegaly.

Key Points:

  • Diagnostic yield: 72%
  • Lymphoma diagnosed: 58.5%
    – 66% B-cell NHL (mainly SMZL, DLBCL)
    – 23% T-cell
    – Subtyping possible in ~88%

Predictors? None.

Age, B symptoms, LDH, splenic size, focal lesions — not reliably predictive.

Risk: 30-day mortality ~5% (sepsis most common)

Even in the PET-CT era, diagnostic splenectomy retains value — but strictly as a last-line diagnostic intervention.”
