Marwa Mostafa: Iron Metabolism in the Differential Diagnosis of Anemia
Marwa Mostafa, Clinical Pathologist at Apollomed Medical Center, shared a post on LinkedIn:
“After classifying anemia by MCV and reticulocyte response…
The next question is:
- Is iron deficient, unavailable… or overloaded?
Understanding iron metabolism is the key.
Here’s a simple clinical approach
Iron deficiency anemia
- Decreased Serum iron
- Increased TIBC
- Decreased Ferritin
Anemia of chronic disease
- Decreased Serum iron
- Decreased / normal TIBC
- Increased Ferritin
Iron is present but not available (hepcidin effect)
Iron overload
- Increased Serum iron
- Decreased TIBC
- Increased Ferritin
Risk of iron deposition – liver, heart, endocrine organs
Clinical tip:
- Ferritin is a storage marker and an acute phase reactant
Always interpret it in clinical context
Understanding iron metabolism simplifies anemia interpretation and helps avoid common diagnostic pitfalls.
