”A patient’s complete blood count (CBC) showed thrombocytopenia.

Clinically, the patient presented with petechiae and epistaxis.

The patient received approximately 12 units of ABO and RhD-compatible platelet transfusions; however, a repeat CBC performed post-transfusion revealed a platelet count that was even lower than the pre-transfusion level.

Further evaluation showed:

  • No evidence of disseminated intravascular coagulation (DIC)
  • No splenomegaly
  • No history of heparin therapy
  • No features suggestive of thrombotic thrombocytopenic purpura (TTP) or immune thrombocytopenia (ITP)
  • No detectable autoantibodies
  • Not on any antiplatelet medications
