Charles Okyere Boadu: What Is The Most Likely Cause of Persistent Thrombocytopenia Despite Transfusion?
Charles Okyere Boadu, Medical Laboratory Science Student at the University for Development Studies, shared on LinkedIn:
”A patient’s complete blood count (CBC) showed thrombocytopenia.
Clinically, the patient presented with petechiae and epistaxis.
The patient received approximately 12 units of ABO and RhD-compatible platelet transfusions; however, a repeat CBC performed post-transfusion revealed a platelet count that was even lower than the pre-transfusion level.
Further evaluation showed:
- No evidence of disseminated intravascular coagulation (DIC)
- No splenomegaly
- No history of heparin therapy
- No features suggestive of thrombotic thrombocytopenic purpura (TTP) or immune thrombocytopenia (ITP)
- No detectable autoantibodies
- Not on any antiplatelet medications
What could be the most likely cause of the persistent thrombocytopenia or the underlying diagnosis?”
